Current Job Opening – Children's HCBS WAIVER Case Manager

Program Expansion - Position Available January 2008

Position Summary

The Case Manager is responsible for providing clinical case management and home-based services to children with emotional and behavioral issues and their families in Westchester County. The Case Manager follows an integrated, strength-based model with strong parental involvement to support community plan. The Case Manager must establish and maintain the highest of professional standards and be willing to work autonomously following a period of training.

Hours of Work

The Case Manager normally works 35 hours per week; actual hours and schedule to be determined by program needs.

Specific Duties

  1. Provide outreach to engage children and families who are eligible for the program.
  2. Conduct a comprehensive, strengths-based assessment of the child and family and their service needs.
  3. Develop a child and family-specific service plan in collaboration with each family and their support circle.
  4. Assist families in securing identified services and advocating to other systems for the provision of these services in an individualized and collaborative manner.
  5. Maintain an up to date and complete client case record and provide other written reports and statistics as required by the Case Management Program.
  6. Facilitate the creation of a support network for each child and family; convene regular meetings of the family and support circle to monitor progress towards jointly developed service goals.
  7. Provide crisis intervention and stabilization to children and families in the program as needed.
  8. Other designated or related duties.

Qualifications

The Case Manager for WAIVER must have a Master’s Degree and two years experience with SED children or a Bachelor’s Degree with a minimum of four years experience with SED children. The Case Manager must have an enthusiasm for home-based service delivery and cross systems collaboration. Additional experience with the OMR/DD population is preferred. Candidates must consent to Fingerprinting and a Criminal History Information Records search. Candidates must have a valid driver’s license and reliable vehicle. He or she must meet MHA guidelines to drive agency vehicles. Candidate must be proficient with Microsoft Office, Word and Outlook.

Reporting Relationship

The Case Manager reports directly to the Children’s Case Management Supervisor.
