Supported HousingMHA’s Supported Housing Program offers case management and in-home support to individuals in recovery from mental illness. The program offers 34 adults (men and women) the opportunity to live the most independent lives possible, living in the community of their choice. Is It for Me?Program Admission Criteria:
What We DeliverMHA has 34 Supported Housing program slots throughout Westchester County. Individuals are given the opportunity to live in their own apartments or in shared apartments with the continued support of case managers. These apartments are permanent in that once a person is admitted to the program they can remain with MHA as long as they need or request the support. We currently own two buildings, one in Yonkers and one in White Plains, which offer apartment style living to 11 individuals. The Supported Housing Program also sponsors 23 individuals in scattered site apartments throughout Westchester County. Program participants receive assistance from a Housing Case Manager, with at least two in-home visits per month, more if needed/requested. The program goal is to assist individuals in living independently and offering continued training around budgeting, landlord negotiations, roommate relationships and accessing community resources. Four of the 34 beds are dedicated to adults 18 to 22 years old. To begin the process of applying for Supported Housing you must first file an application with Westchester County Department of Community Mental Health. To receive a Westchester County Housing Referral Application you may call their office directly at 914-995-5220 or download the Single Point of Entry Application from http://www.westchestergov.com/MentalHealth/. WhereMHA currently offers support throughout Westchester County. We own one building in Yonkers and one in White Plains. The remaining program participants reside in apartments of their choice throughout the county. WhenTwo Case Managers are available to Supported Housing members 35 hours per week. They can be reached via cell phone or office phone. In addition, all program participants have access to a 24-hour emergency line. Languages SpokenServices are available in English and Spanish. Se habla Español. CostSupported Housing residents pay 30% of their income toward rent. The remainder is subsidized through the New York State Office of Mental Health and Westchester County Department of Community Mental Health. Staff/Service ProvidersSupported Housing is staffed by two full-time bachelor’s level Case Managers. The program is supervised by a master’s level Director. Return to the top of the page.
