Assertive Community Treatment (ACT)Assertive Community Treatment (ACT) is a mobile multi-disciplinary team approach that delivers comprehensive and flexible treatment, support and rehabilitation services to individuals with severe mental illness; services are provided in the recipient's natural environment, rather than in an office setting. ACT supports recovery through a highly individualized approach that provides recipients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. It was implemented nationally in the 1980s and it serves individuals with the most serious symptoms of mental illness whose needs have not been well met by more traditional service delivery approaches. Is It For Me?ACT provides services to adults, ages 18 and older, residing in central Westchester County, and who have been diagnosed with a serious mental illness. Services are provided to those individuals who have been unable to benefit from traditional methods of service delivery and who, therefore, require more intensive outreach and mobile supports to achieve sustained stability and progress in their life situation. What We DeliverACT provides treatment, case management, rehabilitation and support services that include visits with the recipient at their home and/or designated neighborhood location. ACT provides participants with the following supports and services:
WhereACT services are provided in the community for residents living in central Westchester County. Services can be provided at the recipient's home or other areas nearby. WhenACT services are available to recipients 24 hours per day, 7 days a week. CostACT services are reimbursed only through Medicaid. Therefore, individuals meeting the Medicaid eligibility criteria will be considered for services and will be assisted to apply for benefits if Medicaid is not already in place. Languages SpokenAll ACT services are available in English. Staff/Service ProvidersThe ACT team is multi-disciplinary and includes a team leader, psychiatrist, two nurses, a family specialist, substance abuse specialist, employment specialist, life skills specialist peer counselor and program assistant.
