We have an exciting new opportunity for a Coordinator in our Care Management program.
The Coordinator is responsible for supervising the daily activities of the care management staff, performing utilization management, care management and quality management activities. The Health Home Coordinator assumes clinical supervisory functions to reach programmatic goals, such as, training, monitoring of records, and evaluation of staff and supervision of care management. He/she assists in the operations of Health Home functions including planning, monitoring, and implementation of care for the client. The Coordinator evaluates the clients’ outcome and guides care managers in the revision of the clients’ care plan as per program requirements. The Coordinator directly interacts with a variety of hospital systems, the medical staff, health plans, and community based services.
Ability to master an electronic record system, meet deadlines and productivity targets and willingness to grow and change are musts.
The candidate must be a licensed LMSW, LMHC, or LCSW. The Care Management Coordinator must have a minimum of two years’ experience working in the social service field. He/she must have minimum of one year of supervisory experience. The Coordinator must have a valid driver’s license, car, and meet MHA guidelines to drive agency vehicles. Basic computer skills are required. Candidates must consent to Fingerprinting and a Criminal History Information Records search.
