We have an exciting new opportunity for a Full-Time Care Manager (CM) in our Health Home Serving Children located in our Tarrytown Office.
The Care Manager will assess, oversee and coordinate a child’s needs including medical, behavioral health, substance abuse, and social services. The Care Manager will provide direct clinical services in the community and/or telephonic direct service work with families. The Care Manager is expected to practice in a recovery oriented, person–centered methodology. The Care Manager must establish and maintain the highest of professional standards and be willing to work autonomously following a period of training.
The CM must have a Bachelor’s Degree with two years of relevant experience or be licensed as a Registered Nurse with two years of relevant experience or a Master’s Degree with one year of relevant experience.
Ability to master an electronic record system, meet deadlines and productivity targets and willingness to grow and change are musts.
The CM must have a valid driver’s license, car, and meet MHA guidelines to drive agency vehicles. Basic computer skills are required. Candidates must consent to Fingerprinting and a Criminal History Information Records search.
