We have an exciting new opportunity for an Individualized Care Coordinator to work in the Cross Systems Unit at the Westchester County Department of Social Services.
The ideal candidate will be an energetic and professional individual who will provide clinical case management and home-based services to children who are emotionally disturbed in Westchester County. He / She follows an integrated, strength-based model with strong parental involvement to support a community plan.
The Individualized Care Coordinator must have a Master’s Degree in a human services field, preferably social work, with at least one year of experience in providing direct services to or in linking children with serious emotional disturbance to a broad array of services OR a Bachelor’s Degree in a human services field and two years of experience. In addition, the ICC should have some training in mental health diagnoses, suicide assessment, psychopharmacology, crisis intervention techniques and available community resources. Must have ability to exercise sound judgment, be compassionate and respectful, and work independently.
The ICC must have a valid driver’s license, car, and meet MHA guidelines to drive agency vehicles. Candidates must consent to Fingerprinting and a Criminal History Information Records search. Ability to master an electronic record system, meet deadlines and productivity targets and willingness to grow and change are necessary.
