We have an exciting new opportunity for a Full-Time Nurse Care Manager.
The Nurse Care Manager is the expert on the Care Management team for the coordination of needed services related to chronic medical conditions. The Nurse Care Manager will serve as a consultant to other team members and may take over some of the work needed to adequately coordinate care if the management of a particular medical condition becomes complex. The Nurse Care Manager will assess client needs and coordinate services designed to address those needs, including primary and specialty care, behavioral health care, substance abuse, and social services. The Nurse Care Manager will coordinate services through face-to-face, telephonic and written communication, both in the community and in the office.
The Nurse Care Manager will have at least an LPN with a minimum of two years of experience working with adults with serious mental illness. Computer skills and recovery oriented practice are required. Current Driver’s License required and driving record must meet requirements for insurability. Candidates must consent to Fingerprinting and a Criminal History Information Records search.
