We have an exciting opportunity for a Full-Time LPN or RN in our Clinics.
The Clinic Nurse is the expert on the team for the coordination of needed services related to chronic medical conditions. The Nurse will serve as a consultant to other team members and may take over some of the work needed to adequately coordinate care if the management of a particular medical condition becomes complex. The Nurse will assess client needs and coordinate services designed to address those needs with primary and specialty care. The Nurse will develop relationships with providers in order to advocate for and insure that the client’s health care needs are adequately addressed. The Nurse must maintain a professional, thorough and accurate documentation in MHA’s Electronic Health Record. The Nurse is expected to know and follow the established policies and procedures of MHA of Westchester and OMH. The Nurse is expected to practice in a recovery oriented, person–centered methodology.
The Nurse Care Manager will have at least an LPN with at least two years of experience working with adults with serious mental illness. Computer skills and recovery oriented philosophy to practice are required.
