Our Mobile Mental Health team provides treatment and care management services in the home or other community location for high-risk children between the ages of five and 18 who have behavioral health conditions and have had difficulty benefiting from traditional behavioral health treatments in a clinic setting. Additionally, our Home and Community Based Services Waiver and Intensive Care Management programs provide community-based care coordination services for children ages five to 17 who have behavioral health issues that have led to psychiatric hospitalization or have put the child at risk for an out-of-home placement. Our individualized recovery-oriented services and multi-disciplinary collaborations help children and families obtain needed services and supports.
