We have an exciting new opportunity for a Peer Outreach Worker/Care Manager in our Tarrytown, NY office in the Certified Community Behavioral Health Clinic program.
SUMMARY of DUTIES
Our Certified Community Behavioral Health Clinic (CCBHC) Program delivers behavioral health services to individuals who are uninsured and underinsured or have otherwise been unable to connect to needed services as well as to members of the military, veterans and their family members. The Peer Outreach Worker/Care Manager is an individual who has direct experience of challenges with Substance Use and/or mental health conditions and who assists with identifying and engaging individuals eligible for our CCBHC series. The Peer Outreach Worker/Care Manager is responsible for 1) assisting to educate the community about these new services; 2) Identifying and engaging with those who are eligible for MHA services, and 2) as appropriate, connecting these individuals to other supportive services including for example, housing, employment, educational supports, physical care, and 3) maintaining contact to assure continuity of services. MHA staff value individual rights of self-determination, choice, shared decision-making and collaboration. When possible we utilize evidence-based practices; always embracing a trauma-informed and person-centered approach.
SPECIFIC DUTIES and RESPONSIBILITIES
The Peer Outreach Worker/Care Manager will employ the principles of Peer Support and have experience providing direct service and intervention to individuals with behavioral health conditions to fulfill the following:
1. Provide culturally sensitive outreach and engagement with individuals who are eligible for CCBHC services.
2. Connect individuals with appropriate CCBHC services, respecting rights of self-determination and choice.
3. Working collaboratively, identify other community services that support recovery and help establish connections to those services.
4. Maintain contact to assist individuals negotiate systems, coordinate multiple aspects of care, and establish connections with additional service providers as needed.
5. Document client interactions in electronic health record to maintain a permanent record of client activity according to established methods and procedures.
6. Participate in agency training programs.
7. Follow the established agency/program policies & procedures.
8. Maintain appropriate professional standards.
9. Other designated or related duties.
REQUIRED KNOWLEDGE and SKILLS
1. Experience providing direct service and intervention to individuals with Substance Use and/or Mental Health challenges.
2. Knowledge of the challenges experienced by and issues pertinent to members of the military, veterans and their family members preferred.
3. Knowledge of the common challenges associated with substance use/mental health conditions.
4. Working familiarity with community resources, including entitlement programs, medical, financial and legal services, housing and emergency food programs, and behavioral health treatment and rehabilitation services.
5. Understand the impact of culture on behavioral health conditions and be able to effectively engage clients in culturally sensitive manner.
6. Must be able to perform duties in an independent and efficient manner.
7. Familiarity with and ability to use an Electronic Health Record system.
8. The Peer Outreach Worker/Care Manager is expected to practice in a recovery oriented, person–centered manner.
1. The Peer Outreach Worker/Care Manager will have training in Peer Support services – certification as a Peer Support Counselor or Certified Recovery Peer Advocate is strongly preferred.
2. The Peer Outreach Worker/Care Manager will have at least two years of experience working in this field.
3. Basic Computer knowledge and ability to document in an electronic health record is required.
4. Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
5. The Peer Outreach Worker/Care Manager must have a valid Driver’s License and a driving record that is satisfactory to our insurance carrier.
6. The Peer Outreach Worker/Care Manager must maintain current New York State Defensive Driving certification.
7. The Peer Outreach Worker/Care Manager must be cleared NYS Justice Center through fingerprinting for a Criminal History Records search.
8. The Peer Outreach Worker/Care Manager must be cleared by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List.
9. The Peer Outreach Worker/Care Manager must be cleared by the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect.
10. The Peer Outreach Worker/Care Manager must be legally eligible to work in the United States.
