Join our Team! We have an exciting new opportunity for a licensed (LCSW, etc.) Program Coordinator in our HCBS/CORE program in Tarrytown, NY. Base Annual Salary: $65,000.
SUMMARY of DUTIES
HCBS/CORE provides opportunities for individuals who are Medicaid beneficiaries with behavioral health conditions/substance use disorders (SUD) to receive services in the home or community. HCBS/CORE utilizes a recovery model of care, which emphasizes and supports a person’s potential for recovery by optimizing quality of life and reducing symptoms of behavioral health conditions and substance use disorders through empowerment, choice, treatment, educational, employment, housing, and health and well-being goals. Recovery is generally seen in this approach as a personal journey rather than a set outcome and one that may involve developing hope, a secure base and sense of self, supportive relationships, self-direction, social inclusion, and coping skills. The Program Coordinator is responsible for the provision and supervision of direct care services in Westchester and Rockland Counties. When possible, the Program Coordinator will use evidence-based practices, always embracing trauma-informed and person-centered approaches.
SPECIFIC DUTIES and RESPONSIBITLIES
1. Responsible for assisting Program Director with program operations including; recruiting, hiring, training, and evaluating of staff, monitoring of revenues and expenditures, compliance with external accountability sources.
2. Provide services to help individuals with behavioral health diagnosis/SUD to achieve stability and functional improvement in the following areas: daily living, finances, housing, education, employment, personal recovery and/or resilience, family and interpersonal relationships and community integration.
3. Provide supervision, mentoring, coaching and performance reviews for direct reports.
4. Coordination and facilitation of team meetings.
5. Network with and provide community education surrounding HCBS/CORE at Care Management Organizations, Managed Care Organizations, and other treatment providers.
6. Outreach individuals for service from Care Managers and other referral sources
7. Conduct HCBS/CORE-specific referral, assessment and intakes with HCBS/CORES candidates.
8. Assist Program Director in assigning referrals for appropriate services with available staff based on assessment/intake.
9. Support the completion of authorization requests, assessments and ISPs. Work with MCOs to troubleshoot any issues in obtaining necessary documentation and authorization responses.
10. Tracking of data including but not limited to referrals, outreach, intake, authorizations, admission, goal setting, and reauthorizations.
11. Modeling and teaching a person-centered approach, honoring choice, self-determination and shared decision-making.
12. Oversight of the EHR/chart and overall record keeping process.
13. Carrying out quality assurance activities including auditing no less than 5% of all progress notes submitted for content.
14. To assume any other responsibilities as required.
REQUIRED KNOWLEDGE and SKILLS
1. Prior knowledge and/or use of Trauma-Informed, Person-Centered Practice and Motivational Interviewing.
2. Knowledge of the recovery process and the ability to facilitate recovery using established standardized mental health processes.
3. Knowledge and skill to teach and engage in basic problem-solving strategies to support individual clients in self-directed recovery.
4. Knowledge of the common challenges associated with mental health conditions.
5. The Program Coordinator must demonstrate positive leadership, a deep belief in recovery and self-determination, flexibility, patience, and effective written and verbal communication skills.
6. The Program Coordinator must be able to perform their duties with minimal supervision and consistently use good judgment.
QUALIFICATIONS
1. The Program Coordinator shall be an experienced Licensed Practitioner of the Healing Arts: Licensed Psychiatrist, Licensed Clinical Social Worker, Nurse Practitioner, Physician, Physician Assistant or Licensed Psychologist and practicing within the scope of their State license.
2. The Program Coordinator must have at least 5 years’ experience in supervisory level positions.
3. The Program Coordinator must demonstrate positive leadership, a deep belief in recovery and self-determination, flexibility, patience, and effective written and verbal communication skills.
4. It is expected that the Coordinator is able to perform their duties with minimal supervision and consistently use good judgment.
5. Eligibility for/certification through the Academy of Peer Specialist (APS) and The Certified Recovery Peer Advocates (CRPAs) is highly desired for this position.
6. Proficiency in computer skills in EHR, Microsoft Office, spreadsheets and data base management are required
7. Basic Computer knowledge and ability to document in an electronic health record is required.
8. Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
9. The Program Coordinator must have a valid Driver’s License and a driving record that is satisfactory to our insurance carrier.
10. The Program Coordinator must maintain current New York State Defensive Driving certification.
11. The Program Coordinator must be cleared NYS Justice Center through fingerprinting for a Criminal History Records search.
12. The Program Coordinator must be cleared by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List.
13. The Program Coordinator must be cleared by the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect.
14. The Program Coordinator must be legally eligible to work in the United States.
