Fax form to (914) 347-8859 attention Central Scheduler. Intakes are processed within 24-48 hours of receipt. Due to high call volume we will relay the appointment information to you as soon as possible via phone. If this form is incomplete scheduling can be delayed.

MHA’s clinics are open Monday through Friday, 9:00 a.m. to 5:00 p.m. as well as evenings.

Phone & Online Help
Our Information and Referral Line - reached at (914) 345-0700 ext. 7303 - provides information about mental health issues as well as resource and referral information to community members, professionals, clients and agencies. We offer information about MHA's services as well as other services in Westchester County and the surrounding area. Our Information and Referral Line is staffed by individuals knowledgeable about Westchester County resources and is supported by a professionally maintained database of services. Professional standards of confidentiality apply to every call. The Information and Referral line is staffed from 9:00 a.m. to 5:00 p.m. Monday through Friday. Voice mail messages and e-mail message to help@mhawestchester.org may be left at any time.

Or even someone who we’ve never met but through connection can help provide a welcome reprieve from our worries and stress.
MHA's Community Connection Support Line is available to all members of our community – at no cost – and is staffed by credentialed professionals Monday through Friday, from 9:00 am to 5:00 pm.
